Monthly Evaluations
Apprentices are required to evaluate themselves and turn in Self-Evaluation forms to the Training Center office for each month and by the 15th of the following month. An Apprentice can he dropped from the program if these forms are more than 2 months behind. Employer Evaluation forms are also required and should be filled out and signed by the jobsite foreman. It is the responsibility of the Apprentice to see that these forms are received by the Training Center.
To qualify for a pay increase, the Apprentice must work a minimum of 750 hours per six-month period and achieve satisfactory progress in skills and satisfactory standings in all categories on the form. Evaluation forms are necessary in determining whether there is satisfactory improvement and also to show if and where more improvements are needed. Mentoring forms will also he reviewed in conjunction with these evaluation forms.

Please fill out Employer forms completely & review it with the Apprentice prior to its return. We encourage comments about the progress of the apprentice. This form must he received by the Training center and is the responsibility of the Apprentice.
